Guidelines for Purchasing Airline Tickets

Purchase a ticket that allows changes but is non-refundable and that allows seat selection at time of purchase.

Individuals who want to arrive a day before the meeting or depart after the last day of the meeting need to send Anne Lageson their proposed flights BEFORE purchasing. If there are less expensive options available for the regular meeting dates, the program will reimburse you for the smaller amount, and you will be responsible for paying the additional cost.

Anne is able to purchase tickets for a limited number of individuals. Please contact Anne with any questions and for more information.

 

Reimbursements

To ensure the smooth processing of your reimbursement request, please ensure that your details are set up on our financial system. To do so, please follow the steps below. If you have questions, please contact Anne Lageson.

  1. Fill out the Personnel Information Form and one of the following - W9 (if a resident of the USA) or W8BEN (if resident elsewhere in the world). Please note that electronic signatures are acceptable.
     
  2. Save files in these formats:
